As per By-Law 1.a: Each Club shall affiliate with the Loddon Valley Football Netball League by signing an Affiliation Form and submit the Affiliation Form to the LVFNL Board Secretary or League Executive Officer prior to the commencement of the current season

This form is to be completed in duplicate prior to the commencment of any game at your home ground. The home club should retain one copy and the other copy is to returned to the league with the match day documents.
